If you ever had to bail off a KISS what was the reason please.

Too much laughing at PDF's, causes mouthpiece leaks ;)

Flood caused by the scrubber tube becoming oval. I’d noticed it becoming harder and harder to put on after filling the scrubber but didn’t twig why.
A new tube fixed the problem, but afterwards I also used inner tube over the top and bottom joins on the scrubber canister as belt and braces.

Seriously:
1. Badly fitted seacure mouthpiece.... got back out and fixed in about 5 mins with some silicone and a whipping.
2. O2 Injection leak, carried on the dive with regular dil flushing....but bailout was close
3. a "friend' dicked about with his o2 valve when the skipper screwed up shotting, then jumped in with it turned off...along with the dil...and 2 bailouts...and a suit bottle. What a prat that 'friend' was ;)

Slow leak through the mouthpiece and the loop went a bit CO2. I think it was after 50min or so, so a decent amount of water probably got in.
Dicking about practicing bailouts, swapping between BOV and a reg over and over. On one bailout I forgot to close the DSV, swapped back on the loop, took a few breaths and then the CO2 went sky high.
Can't think of any others.

Split in the mouthpiece gradualy flooded the unit. All good till I went head down to retrieve the main line reel and got a gob full of caustic liquid in a cave
Bailed to OC via BOV and rinsed my mouth out but it was nasty
